The finished size is about 4" x 4" x 4" but this depends entirely on your yarn choice and tension.

Materials needed:

Green weight 6 yarn (for body)

Red weight 6 yarn (for mushroom hat)

White weight 6 yarn (to embroider spots)

Black weight 4 yarn (to embroider mouth)

6mm crochet hook (or one that matches your yarn and tension) 

12mm safety eyes 

Polyfil (or your preferred stuffing) 

Sewing needle

Darning needle 

Scissors
